No-Shpa is the result of pharmaceutical improvements to papaverine. It was this antispasmodic that was used as an ambulance earlier. Chemical transformations made it possible to obtain the formula of drotaverine - the main active component of the Hungarian drug. It turned out that the derivative is about four times stronger than its “relative”, acts longer and more precisely (directly on the tone of smooth muscles).
The basis of antispasmodic action
Unlike standard painkillers, No-Shpa works against the pathogenesis of spastic pain - it eliminates smooth muscle spasms by inhibiting specific enzymes. Their production may be of inflammatory or neurogenic origin. Enzymatic chains bring the muscles into a state of hypertonicity, in fact - convulsive. Drotaverin interrupts the chain, releasing smooth muscle “out of the paws” of spasm. The soft expansion of blood vessels under the influence of this chemical substance improves the blood supply to tissues, their nutrition with oxygen and other important substances.
Once inside the body through the digestive tract or bypassing it, No-Shpa quickly and completely binds to the protein substances of the blood, spreads throughout the body. With oral administration, the action develops within 12-15 minutes, since the drug is absorbed by 96%. Intravenous administration allows the antispasmodic effect to manifest itself in the second minute after the start of the injection, intramuscular - in five to seven minutes.
Direct indications for use
The main advantage of No-Shpa over other antispasmodics is the minimal side effect in relation to myocardial function. Drotaverin does not affect the production of contractile enzymes in it. Therefore, the drug is not able to significantly change the heart rate. This applies to both the solution and the well-known yellowish tablets.
The main reasons for using injections and tablets are diseases of the gallbladder and urinary organs, as well as some disorders in the digestive tract. Reception "No-Shpy" is appropriate for abdominal pain provoked by the following disorders of the biliary system:
- cholecystolithiasis;
- papillitis;
- cholangiolithiasis;
- cholangitis;
- cholecystitis;
- pericholecystitis.
Provocateurs of spasmodic pain from the urinary system are:
- nephrolithiasis;
- bladder tenesmus;
- urethrolithiasis;
- cystitis;
- pyelitis.
It should be borne in mind that the drug is used as part of the complex therapy of these diseases.Timely and adequate therapy prevents sharp pains in the kidneys (colic). If colic already takes place, an antispasmodic is administered in an increased dose intravenously.
As an auxiliary treatment, “No-Shpa” is used for pain in the abdominal region, provoked by:
- peptic ulcer of the stomach;
- gastritis;
- enteritis;
- colitis;
- diseases of the duodenum.
Other reasons for appointment
In addition to these official testimonies, No-Shpu is also used in other situations. For example, at elevated body temperature, drug combinations are used with antihistamines and analgesics (Diphenhydramine or Tavegil andAnalgin"). As a rule, doctors administer the “triplet”, however, “among the people” they adapted to bring down the temperature using the same name tablets, taking three different names of medicines at once.
In pediatrics
Giving “No-Shpu” to children under the age of six is contraindicated. However, in the case of persistent hyperthermia accompanying viral diseases, emergency doctors can make an injection with the drug to a young child. In this case, reduced dosages of all components of the “triad” are used. After the injection, the temperature quickly drops. The need for an injection is determined by the doctor, taking into account the condition, age of the child, as well as the ratio of risk and benefit.
When treating abdominal pain, the dosage regimen for children is as follows.
- From six years. The maximum daily dosage of No-Shpy is 80 mg. That is, two tablets divided into two doses.
- From 12 years old. Daily dose of 160 mg. It is allowed to give one or two tablets at a time in two doses. That is up to four tablets per day.
In obstetric practice
"No-Shpa" during pregnancy can be assigned to a woman only in the early stages of bearing a child. The goal is to eliminate the tone of the uterus. In this case, the drug is used according to general recommendations - up to six tablets per day. The exact dose should be agreed with your doctor.
The fact that the drug is prescribed by gynecologists for the prevention of miscarriage does not mean that a woman can take it herself. There is evidence of the absence of mutagenic and teratogenic effects of No-Shpa on the fetus, however, taking the drug in late pregnancy is contraindicated. It can provoke relaxation and opening of the cervix, bleeding, up to premature birth.
For headache
The use of “No-Shpy” makes sense only if headaches are triggered by a spasm of cerebral vessels. As a rule, this is preceded by an increase in blood pressure. Taking a drug to treat migraine is also possible, since this pain has a vascular origin. Tablets can be used alone or in combination with analgesics (Aspirin, Analgin, Ibuprofen and their analogues). The drug is dosed according to general rules - up to six tablets per day for an adult.
For toothache
From the point of view of logic, toothache has nothing to do with No-Shpe, since it provokes damage to the dental nerves, an acute inflammatory process. It is more appropriate to take non-steroidal anti-inflammatory drugs ("Ddiclofenac", "Ketorolac").
But popular practice shows that an antispasmodic can be useful if the karyotic cavity has a thin bottom or partially exposes the pulp. Placing a piece of No-Shpy tablet in the tooth has a clear anesthetic effect. There is a feeling of numbness, spreading from the affected tooth to the cheek and tongue. This action is achieved if there is the possibility of direct contact of the tablet and the neurovascular bundle of the pulp.
Doctors do not recommend constantly practicing this method of pain relief. It should be left as a last resort. Use only in the absence of specialized tools at hand, as a way to alleviate pain before a visit to the dentist.
For pressure disorders
The drug "No-Shpa" has a mild vasodilator effect. This indicates the ability of the drug to lower blood pressure to some extent. This property cannot be used for the treatment of hypertension, since for such purposes there are special drugs that are taken for life. But if another increase in pressure provokes a headache, dizziness, nausea, then a pair of No-Shpy tablets can ease the patient’s general well-being and help overcome the hypertensive crisis.
Hypotonic patients should be careful with this drug. Taking the pill, with a tendency to low blood pressure, can provoke:
- dizziness;
- weakness;
- drowsiness;
- lack of appetite;
- tremor of limbs;
- loss of consciousness.
With abdominal pain
It is generally accepted in society that “No-Shpa” helps with any pain localized in the stomach. And doctors allow the removal of abdominal pain with this drug. The medicine should be considered as a means available for home use, allowing you to hold out until the ambulance arrives. Unlike analgesics, it is not able to lubricate the clinical picture of the “acute abdomen”, to dull the symptoms of pancreatitis or appendicitis, thereby complicating the diagnosis. At the same time, the drug partially eliminates acute pain, preventing shock in the patient.
If vomiting, diarrhea, nausea, impaired consciousness or other symptoms of poisoning accompany abdominal pain, a No-Shpy appointment should be followed by a visit to the doctor or a call from the ambulance team.
Ampoules or tablets
For home use, it is more advisable to have No-Shpy tablets in the medicine cabinet. They are produced in different packages - from six to 80. The drug is rapidly absorbed from the digestive tract. One or two tablets are swallowed whole, washed down with a sufficient amount of water.
Injections may be given to people with chronic diseases of the gall, kidney, pancreas, stomach, and intestines. Two dice ampoule contains 40 mg of drotaverine and replaces one tablet. Up to six ampoules can be used per day, making an injection up to three times. A clear solution with a yellowish-green tint, which is clearly visible in the syringe, should not be scary. This is the normal color of the dosage form.
Pricking at home should only be done intramuscularly, in the upper outer quadrant of the buttock. For this purpose, a five-cube syringe is used, since the speed of the drug depends on the depth of the solution.
The risks
“No-Shpy” contraindications include conditions in which changes in vascular tone pose a threat to life. It:
- heart failure with low emission syndrome;
- renal failure;
- liver failure.
Also, the drug should not be taken in case of an allergy to drotaverine or excipients. In the first case, another antispasmodic is selected, in the second - replace the oral form with the injectable one or vice versa.
Due to the lack of objective research data, the drug is prohibited for women with breastfeeding, as well as for children under six years of age.
Side effects of No-Shpy are very rare. According to data recorded during clinical trials, their frequency does not exceed several cases per 1,000 patients. Options for unwanted effects of admission:
- headache;
- dizziness;
- nausea;
- violations of vascular tone;
- decrease in blood pressure;
- constipation.
An overdose condition is considered life-threatening to the patient. With the use of more than 240 mg of the drug, a violation of the conduction of the heart nerve bundle is possible, which is fraught with cardiac arrest. If the dose is accidentally or intentionally exceeded, the patient should rinse the stomach, place it under the close supervision of doctors to maintain the basic functions of the body
Reviews
I had severe pain at the beginning of my second pregnancy. The doctor first prescribed candles with papaverine, but it did not help. Then 2 tons of no-shpu with sharp bouts of pain. It helped, and then it started again. When the pain ceased to pass at all, the no-shpa was canceled and assigned 2 t. 3 s.d. Magne B6 + candles with papaverine. The pain went away after about a week. Total: no-spa - in case of emergency.

I often suffer from headaches. Any change in the weather, and especially abrupt, affects me. It even comes to nausea and vomiting. Real migraine, although there is no such diagnosis. “No-shpa” really helps out. I do not like injections. I’m fooling myself and at the very beginning of the attack I’m going to do without pills altogether, then a “light” pill, then a serious one, but the result is always the same - it is the No-Shpa that helps intramuscularly. I try to always have a couple of ampoules at home. In extreme cases, I use "No-shpu" in tablets. Traveling is always with them.

No-spa is a very good, high-quality drug. Started to use from about 18 years old. Most often used for headaches and during menstruation. This medicine has been very helpful during pregnancy. In the last trimester, headaches, and sometimes colic, began to torment. I was very afraid to take any drug, because I was afraid to harm the baby. After consulting with your doctor, they explained to me that this is an absolutely safe drug for a child. After taking the drug, the cramps very quickly disappeared. There is also a minus to the drug - a bitter, terribly unpleasant taste. But its effectiveness exceeds disadvantages.
